New Canaan's Tele-Health program, a pioneering effort that aims to keep seniors healthy through regular electronic communication with a nurse, enters its third year in 2015 with a goal of nearly 100 participants. The program, which organizers believe is the only one of its kind in the nation, encourages senior residents to regularly check their vital signs, such as blood pressure and heart rate, record those readings every day and then discuss them with a nurse through Skype or FaceTime at least twice a month.
